Privacy of your personal information is an important part of the way our clinic provides and delivers quality dental care to you. We understand the importance of protecting your personal information. We are committed to collecting, using and disclosing your personal information responsibly. We also try to be as open and transparent as possible about the way we handle your personal information. It is important to us to provide this service to our patients.

At Glasnevin Orthodontics, Dr. Declan Ward acts as the Data Controller / Privacy Information Officer.

All staff members who come in contact with your personal information are aware of the sensitive nature of the information that you have kindly disclosed to us. They are all trained in the appropriate uses and protection of your information.

Here we outline what our clinic is doing to ensure that:

Only necessary information is collected about you;
We only share your information with your consent;
Storage, retention and destruction of your personal information complies with existing legislation (GDPR), and privacy protection protocols;
Our privacy protocols comply with privacy legislation, standards of our regulatory body, the Dental Council of Ireland, and the law.

How Our Clinic Collects, Uses and Discloses Patients’ Personal Information
We understand the importance of protecting your personal information. To help you understand how we are doing that, we have outlined below how our clinic uses (and discloses) your information.

Our clinic will collect, use and disclose information about you for the following purposes:

To deliver safe and efficient patient care.
To identify and to ensure continuous high quality service.
To assess your health needs.
To provide health care.
To advise you of treatment options.
To enable us to contact you.
To establish and maintain communication with you.
To offer and provide treatment, care and services in relationship to the oral and maxillofacial complex and dental care generally.
To communicate with other treating health-care providers, including specialists and general dentists who are the referring dentists and/or peripheral dentists.
To allow us to maintain communication and contact with you to distribute health-care information and to book and confirm appointments.
To allow us to efficiently follow-up for treatment, care and billing.
For teaching and demonstrating purposes on an anonymous basis.
To complete and submit dental claims for third party adjudication and payment.
To comply with legal and regulatory requirements, including the delivery of patients’ charts and records to the Dental Council of Ireland in a timely fashion, when required, according to the provisions of the Dentists Act of 1985.
To permit potential purchasers, practice brokers or advisors to evaluate the dental clinic.
To allow potential purchasers, practice brokers or advisors to conduct an audit in preparation for a practice sale.
To deliver your charts and records to the dentist’s medical indemnity insurance carrier to enable the insurance company to assess liability and quantify damages, if any.
To invoice for goods and services.
To process credit card payments.
To collect unpaid accounts.
To assist our clinic to comply with all regulatory requirements.
To comply generally with the law.

What data do we collect?
The types of data we may collect are listed below and we will only use that data in ways relevant to carrying out our legitimate purposes and functions and in a way that is not detrimental to the interests of our patients or staff. Our clinic will take particular care in the collection and storage of any personal sensitive data. All electronic data relating to you is encrypted and is stored on a password protected computer network. Off-site encrypted copies of this data are backed-up remotely each evening by a Data Backup and Recovery Specialist. Everyone working within the clinic has a legal duty to keep information about you confidential. Similarly, anyone who receives information from us has a legal duty to keep it confidential.
We keep records about your health and any treatment and care you receive from our clinic. These records help us to ensure that you receive the best possible care. Some data may be written down in paper records but the vast majority of our data is stored on our computer system. These records may include:
•Basic details about you such as name, address, date of birth, next of kin, etc.
•Contact we have had with you such as previous appointments.
•Notes and reports about your medical history, dental health, treatment history and proposed treatment.
•Digital x-rays and digital clinical photographs.
•Relevant information from people who care for you and know you well such as health professionals and relatives.
•Financial information for payment of any treatments
It is essential that your details are accurate and up to date. Please check that your personal details are correct when you visit us and please inform us of any changes as soon as possible.
How your personal information is used:
Your records are used to direct, manage and deliver the care you receive to ensure that:
•We have accurate and up to date information to assess your oral health and decide on the most appropriate care for you.
•We have the information we need to be able to assess and improve the quality and type of care you receive.
•Your concerns can be properly investigated if a complaint is raised.
•Appropriate information is available if you see another dental professional, or are referred to a specialist.
•From time-to-time, we may use your contact information to send you details of products and services offered in our practices that directly relate to your oral healthcare.
We may also collect technical data about the type of Internet browser and computer operating system that you use. This information does not identify you as an individual and is used only for tracking of site use.
We may disclose your personal information to third parties:
•In the event that we sell or buy any business or assets, in which case we may disclose your personal data to the prospective seller or buyer of such business or assets.
•If Glasnevin Orthodontics or substantially all of its assets are acquired by a third party, in which case personal data held by it about its patients will be one of the transferred assets.
•If we are under a duty to disclose or share your personal data in order to comply with any legal obligation, or in order to enforce or apply our terms of use, and other agreements; or to protect the rights, property, or safety of Glasnevin Orthodontics, our patients, or others. This includes exchanging information with other companies and organisations for the purposes of fraud prevention and credit risk reduction.
Our clinic will not under any conditions supply your insurer with your confidential medical history. In the event this kind of a request is made, we will forward the information directly to you for review, and for your specific consent.

When unusual requests are received, we will contact you for permission to release such information. We may also advise you if such a release in inappropriate.

You may withdraw your consent for use or disclosure of your personal information, and we will explain the ramifications of that decision, and the process.
